Submit Maintenance Ticket

If you believe your Revitalife product isn’t working properly or requires maintenance, please fill out the below form.  A member of our friendly Customer Service team will be in touch with you within seven (7) business days.

Please note that repairs and maintenance of Revitalife products should only be performed by an authorised Revitalife installer.  Any repairs, maintenance, installation, transportation, or opening of the control box, motors or hand controls performed by an unauthorised individual will result in your warranty being voided and is highly discouraged.

If you feel your Revitalife product may be unsafe, lodge a maintenance ticket below and avoid using the product until it has been assessed by an authorised installer.
